In addition to home visits, consultations, and in-patient care, Horizon Hospice sponsored several community outreach programs, both to educate the public about the benefits of hospice and preparing for end of life care as well as programs focused on communities in Chicago to help them cope with loss.
They hosted programs designed to educate Chicagoland about advanced directives, especially living wills. Living wills are legal documents stating the individual's wishes should they become unable to make decisions about their medical care. They also created support groups for grieving family members and close friends of loved ones who had passed away.
These programs and services included the Braveheart in the City program, which organized and encouraged Chicago youth experiencing loss, and a cooking class focused on helping widowers build skills in the kitchen. Some programs were tailored to serve a specific community. These examples and others are included in this section.
